GHUSL : Perform Ghusl at begining and end of night( Note:This Ghusl does not replace Wudhu) Can be with multiple intnentions of Qadr, Tawbah , Ziarat.
CHARITY : Give Sadaqa - Charity - Alms tonight
Keep awake the ENTIRE NGHT without dozing at all and recite as much Salwaat as possible :

اللّهُمّ صَلّ عَلَى مُحَمّدٍ وَآلِ مُحَمّدٍ

allahumma salli `ala muhammadin wa ali muhammadin

O Allah: (please do) bless Muhammad and the Household of Muhammad

Quran and Salwaat Surah 33 (Al‑Ahzaab), Ayah 56 sends blessings to the Prophet (pbuh) :

إِنَّ الله وَمَلائِكَتَهُ يُصَلُّونَ عَلَى النَّبِيِّ يَا أَيُّهَا الَّذِينَ آمَنُوا صَلُّوا عَلَيْهِ وَسَلِّمُوا تَسْلِيمًا

Innal laaha wa malaaa'i katahoo yusalloona 'alan Nabiyy; yaaa aiyuhal lazeena aamanoo salloo 'alaihi wa sallimoo tasleemaa

Verily Allah and His angels send blessings on the Prophet. O you who believe! send blessings on him [Muhammad] and greet him with the fitting salutation.

The best prayer during Qadr nights is to seek forgiveness.
2 Rakat Salaat for Forgiveness of oneself/parents : Offer two Rakats Salat, in every Rakat recite Surah al-Hamd and Sura Ikhlaas X 7 seven times.
After Completion of salaat - repeat 70 Times :

أَسْتَغْفِرُ اللّهَ وَأَتُوبُ إلَيْهِ.

astaghfiru allaha wa atubu ilayhi

I seek the forgiveness of Allah and I repent before Him.

Recite 100 times :

أَسْتَغْفِرُ اللّهَ رَبِّي وَأَتُوبُ إلَيْهِ.

astaghfiru allaha rabby wa atubu ilayhi

I seek the forgiveness of Allah, my Lord, and I repent before Him

Express thankfulness by doing a long Sajdah /Prostration Say : "Azomad dhambo min abdika, fal yohsanal afwu min indika”
Ask for "Health" and "Well being" (Aafiyat) on this night as per Holy Prophet(saw). Recite :

اللهم إني أَسْأَلُكَ العَفْوَ والعَافِيَةَ والمُعَافَاةَ في الدُنْيا والآخِرَة

Allhumma Inni Asaloka Al Afwa wal Afiyata , walmuafata fid-dunya wal akhira

O Allah I ask of you Wellbeing & forgiveness in this world & the hereafter

Recite frequently Dua by Imam Zainul Abideen (a.s) :

ِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ

Bismillaahir Rahmaanir Raheem

In the Name of Allah, the All-beneficent, the All-merciful.

اللَّهُمَّ إِنِّي امْسَيْتُ لَكَ عَبْداً دَاخِراً

allahumma inni amsaytu laka `abdan dakhiran

I have passed the evening as a very humble servant of Yours

لاَ امْلِكُ لِنَفْسِي نَفْعاً وَلاَ ضَرّاً

la amliku linafsi naf`an wa la darran

I have no control over the gains and losses for my self

وَلاَ اصْرِفُ عَنْهَا سُوءً

wa la asrifu `anha su'an

and I cannot drive back any evil from myself.

اشْهَدُ بِذٰلِكَ عَلَىٰ نَفْسِي

ashhadu bidhalika `ala nafsi

I testify to this against myself.

وَاعْتَرِفُ لَكَ بِضَعْفِ قُوَّتِي وَقِلَّةِ حِيلَتِي

wa a`tarifu laka bida`fi quwwati wa qillati hilati

To You do I confess of my powerlessness and my feebleness.

فَصَلِّ عَلَىٰ مُحَمَّدٍ وَآلِ مُحَمَّدٍ

fasalli `ala muhammadin wa ali muhammadin

So, (please do) send blessings to Muhammad and the Household of Muhammad

وَانْجِزْ لِي مَا وَعَدْتَنِي

wa anjiz li ma wa`adtani

and fulfill what You have promised me,

وَجَمِيعَ ٱلْمُؤْمِنِينَ وَٱلْمُؤْمِنَاتِ

wa jami`a almu'minina walmu'minati

as well as all the believing men and women;

مِنَ ٱلْمَغْفِرَةِ فِي هٰذِهِ ٱللَّيْلَةِ

min almaghfirati fi hadhihi allaylati

that is Your forgiveness at this night,

وَاتْمِمْ عَلَيَّ مَا آتَيْتَنِي

wa atmim `alayya ma ataytani

and accomplish that which You have conferred upon me,

فَإِنِّي عَبْدُكَ ٱلْمِسكِينُ ٱلْمُسْتَكِينُ

fa'inni `abduka almiskinu almustakinu

for I am Your slave—the poor, the humble,

ٱلضَّعِيفُ ٱلْفَقِيرُ ٱلْمَهِينُ

aldda`ifu alfaqiru almahinu

the weak, the needy, and the humiliated.

اللَّهُمَّ لاَ تَجْعَلْنِي نَاسِياً لِذِكْرِكَ فِيمَا اوْلَيْتَنِي

allahumma la taj`alni nasiyan lidhikrika fima awlaytani

O Allah, (please) do not make me negligent of Your mention as regards that which You have bestowed upon me,

وَلاَ غَافِلاً لإِحْسَانِكَ فِيمَا اعْطَيْتَنِي

wa la ghafilan li'ihsanika fima a`taytani

do not make me inadvertent to Your favors to me as regards that which You have given me,

وَلاَ آيِساً مِنْ إِجَابَتِكَ وَإِنْ اَبْطَاتْ عَنِّي

wa la ayisan min ijabatika wa in abta'at `anni

do not make me despair of Your responding to me, even if such response is postponed,

فِي سَرَّاءَ اوْ ضَرَّاءَ

fi sarra'a aw darra'a

whether I be in prosperity, adversity,

اوْ شِدَّةٍ اوْ رَخَاءٍ

aw shiddatin aw rakha'in

hardship, comfort,

اوْ عَافِيَةٍ اوْ بَلاءٍ

aw `afiyatin aw bala'in

wellbeing, ordeal,

اوْ بُؤْسٍ اوْ نَعْمَاءَ

aw bu'sin aw na`ma'a

misery, or bliss.

إِنَّكَ سَمِيعُ ٱلدُّعَاءِ

innaka sami`u alddu`a'i

Verily, You are the Hearer of prayers.

Dua by Dua Imam Taqi (a.s) :

ِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ

Bismillaahir Rahmaanir Raheem

In the Name of Allah, the All-beneficent, the All-merciful.

يَا ذَا ٱلَّذِي كَانَ قَبْلَ كُلِّ شَيْءٍ

ya dha alladhi kana qabla kulli shay'in

O He Who has existed before all things,

ثُمَّ خَلَقَ كُلَّ شَيْءٍ

thumma khalaqa kulla shay'in

then created all things,

ثُمَّ يَبْقَىٰ وَيَفْنَىٰ كُلُّ شَيْءٍ

thumma yabqa wa yafna kullu shay'in

and then stays while all things vanish!

يَا ذَا ٱلَّذِي لَيْسَ كَمِثْلِهِ شَيْءٌ

ya dha alladhi laysa kamithlihi shay'un

O He Whose like is not found at all!

وَيَا ذَا ٱلَّذِي لَيْسَ فِي ٱلسَّمَاوَاتِ ٱلْعُلَىٰ

wa ya dha alladhi laysa fi alssamawati al`ula

O He neither in the highest heavens

وَلاَ فِي ٱلأَرَضِينَ ٱلسُّفْلَىٰ

wa la fi al-aradina alssufla

nor in the lowest layers of the earth,

وَلاَ فَوْقَهُنَّ وَلاَ تَحْتَهُنَّ

wa la fawqahunna wa la tahtahunna

nor above them, nor beneath them,

وَلاَ بَيْنَهُنَّ إِلٰهٌ يُعْبَدُ غَيْرُهُ

wa la baynahunna ilahun yu`badu ghayruhu

nor there is between them any god that is worshipped save Him.

لَكَ ٱلْحَمْدُ حَمْداً لاَ يَقْوَىٰ عَلَىٰ إِحْصَائِهِ إِلاَّ أَنْتَ

laka alhamdu hamdan la yaqwa `ala ihsa'ihi illa anta

To You be the praise that none can count save You.

فَصَلِّ عَلَىٰ مُحَمَّدٍ وَآلِ مُحَمَّدٍ

fasalli `ala muhammadin wa ali muhammadin

So, please bless Muhammad and the Household of Muhammad

صَلاةً لاَ يَقْوَىٰ عَلَىٰ إِحْصَائِهَا إِلاَّ أَنْتَ

salatan la yaqwa `ala ihsa'iha illa anta

with blessings that none can count save You.

Day & Night Dua from Imam Sadiq (a.s) :

ِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ

Bismillaahir Rahmaanir Raheem

In the Name of Allah, the All-beneficent, the All-merciful.

اللّهُمَّ إنِّي أَسْأَلُكَ أَنْ تَجْعَلَ فِيمَا تَقْضِي وَتُقَدِّرُ

allahumma inni as'aluka an taj`ala fima taqdi wa tuqaddiru

O Allah :I beseech You that You,

مِنَ الأَمْرِ الْمَحْتُومِ فِي الأَمْرِ الْحَكِيمِ

min al-amri almahtumi fi al-amri alhakimi

while taking decisions and rendering possible things and events decisively

مِنَ الْقَضَاءِ الَّذِي لا يُرَدُّ وَلا يُبَدَّلُ

min alqada'i alladhi la yuraddu wa la yubaddalu

(decisions which can not be changed or bypassed),

أَنْ تَكْتُبَنِي مِنْ حُجَّاجِ بَيْتِكَ الْحَرَامِ

an taktubani min hujjaji baytika alharami

write my name in the list of those pilgrims of Your Holy House

الْمَبْرُورِ حَجُّهُمُ،

almabruri hajjuhum

whose pilgrimage receives Your approval,

الْمَشْكُورِ سَعْيُهُمُ،

almashkuri sa`yuhum

whose efforts are appreciated,

الْمَغْفُورِ ذُنُوبُهُمُ،

almaghfuri dhunubuhum

whose sins are forgiven,

الْمُكَفَّرِ عَنْ سَيِّئَاتِهِمْ،

almukaffari `an sayyi'atihim

whose wrongdoings are overlooked;

وَأَنْ تَجْعَلَ فِيمَا تَقْضِي وَتُقَدِّرُ

wa an taj`ala fima taqdi wa tuqaddiru

that You, while making decisions and rendering possible things and events;

أَنْ تُطِيلَ عُمْرِي فِي خَيْرٍ وَعَافِيَةٍ،

an tutila `umri fi khayrin wa `afiyatin

make possible for me long life, goodness, welfare,

وَتُوَسِّعَ في رِزْقِي،

wa tuwassi`a fi rizqi

and enough means of livelihood,

وَتَجْعَلَنِي مِمَّنْ تَنْتَصِرُ بِهِ لِدِينِكَ

wa taj`alani mimman tantasiru bihi lidinika

and let me be like him whom You helps with Your Religion;

وَلا تَسْتَبْدِلْ بِي غَيْرِي.

wa la tastabdil bi ghayri

not substituting any other in my place.

Recite 100 Rakat Salat. In each Rakat Surah al-Hamd + 10 times Sura Ikhlaas
Gift the thawab to all Masoomeen and all Momineen .Forgive all those Momineen who have wronged you. Allah (swt) will forgive your sins.
